About your visit – We want to answer as many of your questions as we can during your visit. Please prepare for this by bringing:
- Health Insurance Card/Information
- If needed, a friend or family member to take notes/help with questions
- List of all medications you are taking
- List of your questions /concerns
- Computer disk of your MRI or CT scans (not the report-please bring the actual images on disk.)
- Copies of records from doctors related to your visit with us
